Program Title: The Bridge to Self-Sufficiency & Leadership Curriculum

Organization: Redirection Community Outreach

Target Audiences: Homeless/At-Risk Veterans & Underprivileged Youth (San Joaquin County)

Program Overview

This mobile, field-based curriculum uses a trauma-informed, peer-supported model to bridge the gap between crisis survival and long-term stability. Recognizing that homeless veterans and disadvantaged youth face distinct structural and psychological barriers, the curriculum is divided into two specialized tracks anchored by shared core life skills.

Core Tracks & Modules

Track 1: Veterans Resettlement & Stability (Focus: Homeless & At-Risk Veterans)

  • Goal: Navigate complex veteran systems, secure permanent housing, and restore personal independence.
  • Key Modules:
    • VA & CalAIM Benefits Navigation: Direct assistance mapping VA healthcare, HUD-VASH housing vouchers, disability claims, and Medi-Cal managed care supports (HPSJ).
    • Financial Readiness & Stability: Managing credit, avoiding housing scams, budgeting fixed or transitioning incomes, and managing back debts.
    • Peer Support & Whole Health: Addressing isolation, transition stress, and linking to local mental health or substance use care coordination.

Track 2: Youth Empowerment & Mentorship (Focus: Underprivileged & At-Risk Youth)

  • Goal: Intercept generational cycles of poverty or instability through education, practical life mapping, and positive mentorship.
  • Key Modules:
    • Life-Skills & Independent Living: Essential budgeting, household management, nutrition, and personal organization.
    • Educational & Career Pathways: Job readiness coaching, resume building, interview preparation, and connecting with local vocational or continuation education programs.
    • Civic Engagement & Leadership: Mentorship circles connecting youth with positive community role models, local government resources, and conflict-resolution strategies.

Delivery Method

  • Field-Based & Mobile: Delivered directly where participants reside or gather (e.g., community centers, designated outreach sites, or partner church facilities) to eliminate transportation barriers.
  • Warm Hand-Off Approach: Integrated tightly with local safety nets, county social services, and coordinated entry networks to ensure education immediately translates into tangible housing, healthcare, or career placement.

Key Community Resources to Leverage

To maximize reach and connect participants to comprehensive care, Redirection Community Outreach aligns with vital local networks and infrastructure:

  • Managed Care & Health Plans: Partnering with systems like the Health Plan of San Joaquin (HPSJ) to integrate community supports, providing direct pathways for Medi-Cal beneficiaries to access housing navigation and care coordination.
  • San Joaquin County Coordinated Entry System (CES) & 2-1-1: Utilizing 2-1-1 San Joaquin and local Continuum of Care (CoC) portals to cross-reference emergency shelter availability, food distributions, and housing placement pipelines.
  • County Behavioral & Social Services: Collaborating with county programs—such as the SJQ Behavioral Health Homeless Outreach Team and the Human Services Agency—for mental health linkages, supportive services, and IHSS navigation.
  • Local Educational & Youth Networks: Teaming up with local school districts, youth centers, and mentorship groups to intercept at-risk youth with educational support, positive reinforcement, and practical life-skills workshops.

How to Get Connected with the Community

Building deep, trusted roots in San Joaquin County relies on proactive, grassroots engagement:

  1. Direct Field Outreach & Mobile Presence: Deploying teams directly into local neighborhoods, public resource fairs, and community spaces to build trust face-to-face with homeless individuals and at-risk youth.
  2. Coalition and Municipal Collaboration: Engaging with local leadership (such as Stockton City Council and community office tables like District 6) and participating in local safety, health, and youth-advocacy roundtables.
  3. Establishing Warm Hand-Off Pathways: Partnering with established local safety-net providers, churches, and community-based organizations (CBOs) to create seamless referral loops so participants experiencing housing or social crises easily find our programs.
